This page contains information about the whistleblower disclosure process via this disclosure form (other ways of making a protected disclosure are described in your organisation’s Protected Disclosures (Whistleblower) Policy.
Located at the bottom of the page is a link to the Whistleblower form that you will use to report the Wrongdoing/Concern/Issue (Issue).
Some whistleblowers wish to remain anonymous.
Option to maintain anonymity
When submitting a whistleblowing disclosure, there are two types of anonymity. Please carefully read the following which describes these:
- Remaining entirely anonymous, even to the whistleblowing call takers. Such anonymity can impede investigations if there is not enough information to proceed and no way to reach the discloser for further clarification; and
- Remaining anonymous to your organisation, but providing your details to Incident Response Solutions (the external organisation). This will allow us to relay any messages back to you.
If you wish to remain entirely anonymous, please adhere to the following guidelines:
- do not submit/disclose any information that may identify yourself such as your name, job title or location; and
- use a personal device not a work device.
To submit your whistleblowing disclosure using this form, firstly click on the link below. You will then be directed to a form which is managed by Incident Response Solutions, hosted in the Google Suite.
When you complete the form, it will be sent to Incident Response Solutions who will process it in accordance with your organisation’s agreed procedures.
Following your disclosure:
In the event you require a follow up to your disclosure, but have not submitted your contact details, please contact the Whistleblower service at Incident Response Solutions within 10 days.
Your organisation encourages you to make contact with Instep EAP for wellbeing support after you have made a disclosure if you feel that would be beneficial (contact details are provided on your organisation’s Intranet).
About the Whistleblower tool
Incident Response Solutions provides Whistleblowing services to a number of New Zealand organisations. The tool conforms to the requirements of the New Zealand Protected Disclosures (Protection of Whistleblowers) Act and the definition of ‘Serious Wrongdoing’.
The tool is hosted in the Google Suite, which offers advanced security and privacy settings. The information you disclose is only available to authorised Incident Response Solutions staff who must firstly enter their login credentials and pass the two factor authentication (2FA) checks, before being approved to access.
If you submit a disclosure using the tool and accidentally include your name (or provide other information that identifies you), please notify Incident Response Solutions by submitting another disclosure outlining the issue.
